  • Flexible Benefit Levels: Choose benefit amounts ranging from $2,500 to $17,500.
  • Progressive Benefits: Includes decreasing deductibles and increasing ambulance and medical equipment benefits over three years.
  • Cash Benefits: Paid directly to policyholders, regardless of other medical coverage.

Coverage Details:

Accident Medical Expense Coverage:

  • Covers accident-related medical costs, including ambulance, physical medicine, and durable medical equipment.
  • Includes accidental death and dismemberment benefits.
  • Deductibles reduce yearly: $250 in Year 1, $150 in Year 2, and $0 in Year 3.

Cancer and Heart/Stroke Rider:

  • Lump-sum benefits for first-ever diagnoses of covered cancers or heart conditions.
  • Waiting periods apply (90 days for cancer, 30 days for heart/stroke).
  • Coverage for primary policyholders aged 18–64 and dependents up to age 25.

Sickness Hospitalization Rider:

  • Pays lump-sum benefits for the first covered day of hospital admission due to sickness.
  • No deductible and no network restrictions.
  • One benefit per person annually, up to two benefits per family.

Limitations and Exclusions:

  • Pre-Existing Conditions: Coverage excluded for conditions treated or diagnosed within specific waiting periods.
  • Excluded Services: Behavioral health treatments, cosmetic surgeries, and conditions resulting from hazardous activities or occupations.
  • Restricted Benefits: Riders unavailable in certain states, and age limits apply (renewable up to age 65 for riders).
